my opinion is that lenny's wife is not dead. just as there is a flash of lenny in the asylum, there is also a longer flash of lenny lying in bed with his wife, BUT he is wearing the blue shirt he wears through the movie and has tattoos on his chest, implying that it happened after the events of the movie (i.e. it's a flash forward). what is very important is that on his heart where he points in that one picture is now a tattoo that says "i did it".
lenny didn't kill his wife because she isn't diabetic. teddy had to have been lying about that, because lenny would remember this, it having happened before the incident. teddy therefore is still lying, but isn't the murderer at least. somehow lenny was misled (wouldn't be too hard to happen to him after all...) possibly by teddy for his own selfish reasons and he was off looking for a john g that didn't exist. after he kills teddy, he takes a picture, so from that time on he'll think he's finished the job. he gets the tattoo so he'll remember when he sees the other tattoos that he doesn't need to find the murderer anymore. he goes home (he'll remember where that is obviously) and his wife is there. then he's committed for committing those murders.
ok, that's it. i'd say more but i'd rather leave it to you to let it fit in with the rest of the story (me and my friends have found no flaws so far...) please let me know what you think about this explanation!
